I understand that as a volunteer I may be involved in physical activities that have the potential risk of injury. I assume that risk. I will perform only the duties I am comfortable doing. I will not hold the Arizona Diaper Bank or Elontec responsible for any damages or injury to my person or property as a result of my volunteering with the agency.

For privacy purposes, I also understand the Arizona Diaper Bank is a secure facility. I will not take any photos without permission. I understand whatever I see or hear about a family is confidential. I grant the Arizona Diaper Bank and Elontec permission to take photographs and/or video of me in connection with the below-identified volunteer project and to use and publish such photographs in print and/or electronically.
